Setting an Appointment

Call our office at (918) 743-3224 during office hours. Leave a message or speak to Janette to give us information about your needs so we can best direct you.

Click Patient Forms to print the forms that Janette recommends for you. Be sure to sign the HIPAA consent form.

On the day of your appointment, bring completed forms with you. Try to arrive a few minutes early so we can process and gather any information we may need.
